Most teams will always put their best athletes at middie, they can control the game that way. I think as teams get older, more and more top players get moved to defense.
I'd argue the best teams do that now. For me the biggest difference between M&D/Heros/BLC and the next tier (Pride, MDU, NEMS, LBC). is the quality of their defense and the speed and quality of their clears.
